

COVID-19 Information & Policy
We take the health/safety of our clients, providers and business partners seriously. Therefore, we've adopted a COVID-19 (Coronavirus/SARS CoV-2 and related airborne illness) Policy. Please review this Policy carefully, as its acceptance is a condition of Services.

This Policy sets forth measures for the prevention/mitigation of COVID-19 and related illnesses in the conduct of our business. We have established this Policy in conformity with the COVID-19 guidelines/recommendations of the U.S. Centers For Disease Control and Prevention (CDC), as in effect as of 8/1/22 and upon which this Policy exclusively relies.

All with whom we conduct business (including, but not limited to Clients, customers, providers, facility staff/personnel or others) are subject to this Policy. It is the individual responsibility of each party to comply with CDC guidelines, review applicable/relevant state/local guidelines, if mandated, to determine the level of conduct required for compliance in the specific locality where services are to be delivered, and to meet/exceed CDC/State/Local Guideline Standards in effect when Services are rendered. ​

CLIENTS who receive our Services (or their guardian/legally responsible party) agree to:

Cooperate with any routine screening, if/when recommended by state guidelines or facilities/venues (where Services are to be delivered) in conjunction with service appointments

Observe any mandated CDC prevention guidelines, as located at https://www.cdc.gov/coronavirus/2019-ncov/prevent-getting-sick/prevention.html and as may be updated from time to time, during service appointments

Observe appropriate masking and/or social distancing during Service delivery, if recommended/mandated by applicable policies
SERVICE PROVIDERS, including but not limited to those who provide direct/one-on-one companionship/visitation services, and any independent contractors, if applicable, who may deliver Services on our behalf, agree to:

Observe CDC guidelines, as may be in effect at the time of Services (and updated from time to time), as located at: https://www.cdc.gov/coronavirus/2019-ncov/prevent-getting-sick/prevention.html

Comply with any CDC mandates as may relate to vaccination status/medical exception and provide proof upon request

Review CDC/state/local COVID-19 requirements, as updated
​​

Implement measures for self-protection and that of others, before/during/post services
​​

Participate in any screening measures required by facilities/venues
​​

Self-screen/self-test/self-govern (as applicable) prior to client appointments

If sick, stay home, isolate/quarantine and/or seek medical advice/treatment, etc., as warranted and notify Meriatrics LLC immediately for cancellation of service appointment

Notify Meritrics LLC for rescheduling, if upon arrival at a service appointment, a Client or facility/venue (where Services are to be provided) opts to decline Services due to a COVID-related concern.

Service providers are also subject to Covid-related requirements of the U.S. Occupational Health and Safety Administration (OSHA), as may apply to their respective services and any subcontractors/employees/personnel and located at: https://www.osha.gov/coronavirus.

Facilities/Venues
Meriatrics LLC and its Service providers are subject to the COVID (or other related infectious disease) policies of facilities/venues where Services may be delivered, including but not limited to nursing homes, medical facilities, senior residences and other locations. Clients (their guardian/legally responsible party) understand and agree to any rescheduling or cancellation of Services, as may be required in accordance with this Policy.

​​Meriatrics LLC and its Service providers reserve the right to refuse, modify, cancel, postpone and/or reschedule services if, in their sole opinion, doing so is warranted for the potential health/safety of others. We may also change/modify this Policy at any time, for any reason.

COVID-19 (and related illnesses) can pose serious health risks. While this Policy is intended to help mitigate virus spread, we are unable to make any guarantees that you or others will not become exposed/infected. Parties subject to this Policy accept all inherent risks and acknowledge that all risks incurred or suffered are personally/individually assumed, and not the responsibility of Meriatrics LLC.
