

COVID-19 Policy

We take the health/safety of our clients, providers and business partners seriously. So we've adopted this comprehensive policy for COVID-19 and related airborne illnesses. Please review it carefully, as its acceptance is a condition for doing business with us.

This Policy sets forth measures for the prevention/mitigation of COVID-19 and related illnesses in the conduct of our business. All with whom we conduct business, including, but not limited to, clients, customers, providers, independent contractors and/or others, are subject to this Policy.
Our Policy conforms to the guidelines/recommendations of the U.S. Centers For Disease Control and Prevention (CDC). Current CDC guidance and recommendations are located here. Each party is responsible for complying with (or exceeding) mandated CDC guidelines, as well as any state/local requirements, as may be in effect at the time and location of services to be provided. Meriatrics LLC Providers agree to:
​
- 
Observe CDC guidelines/recommendations currently in effect when services are rendered
 
- 
Comply with any CDC mandates as related to vaccination status/medical exception
 - 
Provide proof of vaccination/medical exception upon request
 - 
Self-screen/self-test/self-govern prior to client appointments
 - 
Implement measures for self-protection and that of others, before/during/post services
 
- 
Participate in required screening measures, if applicable, at facilities/venues
 - 
If sick, stay home, isolate/quarantine and/or seek medical advice/treatment, as warranted
 - 
Notify Meriatrics LLC immediately for cancellation of service appointment due to illness
 - 
Notify Meritrics LLC for rescheduling*, if upon arrival at a service appointment/facility. concerns about potentially infectious conditions warrants cancellation
 - 
Comply with any related requirements of the U.S. Occupational Health and Safety Administration (OSHA), as located at: https://www.osha.gov/coronavirus and may apply.

Customers/Clients agree to:

- 
Self-screen/self-test/self-govern, as warranted, prior to client appointments
 - 
Cooperate with any routine screening, if required by CDC/state guidelines
 - 
Observe CDC prevention guidelines/recommendations during visits, when warranted
 - 
Comply with appropriate masking/social distancing, if requested
 
​Facilities/Venues (including staff/personnel at nursing homes, medical facilities, senior residences and other venues where we deliver services or group programs) agree to:

- 
​Provide Meriatrics/providers with an advance copy of COVID-19 related policies, if applicable
 - 
Review facility/venue policies with providers upon arrival, as warranted
 
- 
Screen providers/independent contractors upon arrival, if warranted/required)
 - 
Request cancellation of service(s) if provider/contractor screening is unsuccessful
 - 
Monitor provider/contractor compliance while onsite

Meriatrics LLC reserves the right to refuse, modify, cancel, postpone and/or reschedule services if, in our sole opinion, doing so is warranted for the potential health/safety of others. We may also change/modify this Policy at any time, for any reason.

COVID-19 (and related illnesses) can pose serious health risks. While this Policy is intended to help mitigate virus spread, we are unable to make any guarantees that you or others will not become exposed/infected. Parties subject to this Policy accept all inherent risks and acknowledge that all risks, incurred or suffered, are personally/individually assumed and not the responsibility of Meriatrics LLC.
