​​Meriatrics LLC ("Company", "we") provides a variety of social support services ("Services") for older adults (“Clients”). Detailed information about Services can be found on the Company’s website at: https://meriatrics.com/ (“Website”).

This policy (“Service Policy”, “this Policy”) sets forth the terms, conditions, limitations, exceptions, and exclusions that apply to the Company’s Services and provision thereof.
​
All visitors/users of the Company’s Website, individuals inquiring about Services, prospective and active Clients, and respective authorized parties/signatories acknowledge their review, understanding and acceptance of this Service Policy. They further consent to be bound by this Policy in conjunction with the Company's Terms of Service, Privacy Policy, Disclaimer, and Covid-19 Policy.
​No Medical Advice or Information
​
​All Services are non-medical/non-clinical. No medical, clinical, home health, personal care or other medical/clinical services are offered, implied, or provided.
​
​Anything contained, expressed or implied on our website or in Company correspondence such as emails, advertising materials, service brochures, or other written materials, or, which is imparted during screening, consultations, or the course of doing business, is intended only for informational purposes. No information provided is intended, nor should it be construed, as medical advice under any circumstances. All medical issues and sensitive/confidential matters related to medical issues should be referred to appropriate health care professionals.

Service Providers
​
Meriatrics LLC providers are not licensed or permitted to provide medical, clinical, home health care or personal care in conjunction with the Company’s Services.

​Consultations
​
​A (complimentary) 30–60-minute consultation is required for consideration of Services. Clients, prospective Clients, and respective representatives are required to provide accurate information to the Company during consultations/Services, including disclosure of any Client restrictions, medical or otherwise, as may affect Services or participation in Optional Activity, Escorted Outings, and/or Transportation.

Company Discretion
​
​Service requests are subject to Company approval. We reserve the right to decline/deny or withhold/terminate Services if, in our sole and absolute discretion, we believe that providing/continuing Services would compromise safety, constitute a conflict of interest, violate our policies, or adversely affect the Company, its reputation, Clients, or providers.

Service Agreements
​
​A written service agreement ("Service Agreement"), signed by and between Meriatrics LLC and the Client/authorized representative (“Authorized Party”), is required for Services.
Service Area
​
​We serve Southern New Hampshire/Maine and the Merrimack Valley of Massachusetts. (“Service Area”). Tolls and/or mileage surcharges may apply for travel to service locations beyond a twenty-five (20) mile radius of Hampton, NH (“Local Service Area”).
​
​Service Offerings
​
We offer private visitation and companionship with three (3) service options:
In-person (“Direct Services”); via computer (“Virtual Services"); via phone ("Remote Services”).
​
Note: Client/Authorized Party is responsible for all set up, devices and equipment, mobile/Internet charges, technical issues/problems, and costs on Client side for Virtual/Remote Services.
​
Optional Activity (“Optional Activity”) is included with all service options.
​
Escorted Outings (“Escorted Outings”) are available with Direct Services only and include a companion escort and motor vehicle transportation (“Transportation”).
​
Optional Activity​
Optional Activity is based on Client interest/preference and subject to provider willingness and provisions of this Policy.
​
Clients receiving Direct Services must be able to move/ambulate with minimal assistance to participate in Optional Activity.
​
We reserve the right to modify, limit, or exclude Optional Acitivity if, in the sole and absolute discretion of the provider, such Optional Activity could pose a potential safety risk for the Client or provider.
​
Optional Activity with Virtual/Remote Services is limited and may require modification.
​
Escorted Outings
​
​​​Escorted Outings are available with Direct Services and may be chosen as an Optional Activity subject to a two (2) hour minimum (inclusive of door-to-door transportation).
​
Two (2) hour service minimums apply to Escorted Outings.
Clients must be able to move/ambulate with minimal assistance to participate in Escorted Outings.
​
Clients and escort providers are each responsible for their respective expenses.
​
Providers may temporarily fund nominal expenses on Client behalf if Client is unable. Receipts will be submitted for billing to Client/Authorized Party. Client/Authorized Party agrees to pay all such expenses.
​
All provisions of the “Optional Activity” and “Transportation” sections of this Policy apply to Escorted Outings.
​
​Transportation ​
Motor vehicle transportation is provided only in conjunction with and as incidental to Services. No general taxi, livery, rideshare, or other transportation services are offered, provided or implied.
​
Clients must be able to move/ambulate sufficiently and with minimal assistance to participate in Transportation-related Services.
​
​​All provisions of the “Escorted Outings” and “Optional Activity” sections of this Policy apply to Transportation.
Service Rates
​
​Services are billed by the hour, based on Service option. One-hour minimums apply; partial hours billed in 15-minute increments after first hour. Rates are subject to change. 10% off Services for veterans. Other discounts may apply.
Standard Service Rates:
​
$40/hour - Direct Services
$40/hour - Escorted Outings
$30/hour - Virtual Services
$20/hour - Remote Services
​
Surcharges may apply for tolls and/or travel (.50/per mile) over forty (40) miles round-trip.
​
​​Billing & Payment
​
​Services are currently private pay. We accept cash/ checks made payable to Meriatrics LLC.
​
Payment is expected at time of Service, unless other arrangements are approved in advance. Standing Services (weekly/monthly) are billed monthly.
​
Payment is due within thirty (30) days of billing date. Interest on unpaid balances is charged at a rate of 1.5% per month.
​
​Cancellation Policy
​
​Services may be cancelled by Client/authorized party at any time by email notification to info@meriatrics.com. If less than 72-hour notice (before Client’s next scheduled Services) is provided for cancellation of Services booked in advance, a 15% penalty will be added to final billing amount for unpaid Services rendered through date of cancellation.
​
​Emergencies
​
​In an emergency, as determined by and in the sole and absolute discretion of the provider, 911 assistance will be sought first, followed by notification of Client’s emergency contact. For Services rendered in a care facility, the emergency will also be reported to facility staff.
​
​Providers are not permitted to drive a Client to the hospital or administer medication.
​
Prohibitions
​
​The following are, in the sole and absolute discretion of the Company/provider, causes for postponement, cancellation and/or denial of Services:

Smoking during Services (including tobacco products/vaping) ​

​Aggressive/threatening Client behavior; abusive Client language

Appearance of Client illness or impairment by drugs or alcohol

Client refusal to wear a seatbelt during Transportation
-
Client refusal to comply with provider requests/Company policies
-
Provider concerns as to Client or provider safety
We also reserve the right to postpone/cancel Services due to inclement weather, hazardous driving conditions, unsafe conditions at a service location (uncleared snow, ice, or debris), or unforeseen emergencies as determined by the Company/provider.

NO WARRANTIES OR LIABILITY
​
Clients/Authorized Parties assume all risks associated with Services, Optional Activity, Escorted Outings, and Transportation, including sole risk, responsibility and liability for illnesses/injuries/damages arising from/during the provision of Services or related travel. They further agree that under no circumstances shall Meriatrics LLC be liable for any direct, indirect, incidental, consequential, punitive, special exemplary, or any other damages arising out of our Services or use thereof.

​Meriatrics LLC reserves the right to change/modify this Service Policy at any time.
​
Written notice of revisions to this Service Policy shall be provided in advance to current Client/Authorized Parties. Alternatively, revisions to this Policy, as and when posted on our Website, shall constitute notice and take effect immediately.
