Meriatrics LLC, a Limited Liability Company located in Hampton, NH, USA ("Company", "We", "Our"), provides social support services for older adults ("Clients"), including social visitation/companionship, optional activity, escorted outings and personalized assistance services ("Services"), as detailed on our website https://www.meriatrics.com/ (“Website”).
This policy sets forth the terms, conditions, limitations, exceptions, and exclusions that apply to our Services and provision thereof (“Service Policy”, “this Policy”).
This Service Policy, which applies to all who inquire about, request or receive Services, including but not limited to prospective and active Clients and their respective family members, authorized parties and/or agents, should be carefully reviewed. Acceptance of this Policy, in conjunction with the Company's Terms of Service, Privacy Policy, Disclaimer, and Covid-19 Policy, is a condition of Services.
No Medical Advice or Information
All Services are non-medical/non-clinical. No medical, clinical, home health, personal care or other medical/clinical services are offered, implied, or provided.
All information contained, expressed or implied on our Website, conveyed in Company correspondence, advertising materials, service brochures or other written materials, or imparted during screening, consultations, provision of Services, or any other manner, is intended for general informational purposes only. No information is intended as medical advice, nor should it be construed as such. All medical matters should be referred to qualified medical professionals.
Companion Providers
Those providing Services on behalf of the Company ("Companion Providers", "Companions") are not authorized to provide medical, clinical, home health care or personal care in conjunction with any Company’s Services.
Consultations
A complimentary consultation (30-60 minutes) is required for consideration of Services. Prospective/active Clients, respective family members, and authorized agents, are required to provide accurate information to the Company, including disclosure of any restrictions that might affect Services or Client participation therein.
Company Discretion
All Service requests are subject to Company approval. We reserve the right to decline/deny or withhold/terminate Services if, in our sole and absolute discretion, we believe that providing/continuing Services would compromise safety, constitute a conflict of interest, violate any Policy, or adversely affect the Company, its reputation, Clients, or Companion Providers.
Service Agreements
A written service agreement ("Service Agreement"), signed by and between Meriatrics LLC and the Client and/or a duly authorized family member/agent (“Authorized Party”), is required prior to commencement of Services.
Service Area
We serve locations in New Hampshire, Massachusetts, and Maine. For service locations beyond a twenty (20) mile radius of Hampton, NH ("Local Service Area"), tolls and/or mileage/fuel surcharge (50-cents/mile) may apply.
Services
Service options include the following:

One-on-one visitation and companionship, delivered in-person ("Direct Services"), virtually via Zoom ("Virtual Services") or remotely via phone ("Remote Services")

Optional activity, based on Client interests ("Optional Activity")

Escorted outings to places of interest in the community ("Escorted Outings")

Personalized concierge assistance services, with or without Client accompaniment ("Pal Friday Services")
Escorted Outings and Pal Friday Services are available only with Direct Services. Door-to-door motor vehicle transportation ("Transportation") and Companion Provider are included.
Client/Authorized Parties are responsible for all Client-side set up and costs associated with Virtual and Remote Services, including devices, equipment, mobile/Internet charges, and technical issues.
Optional Activity
Clients must be able to move and ambulate with minimal assistance to participate.
Optional Activity is subject to Company approval, Companion Provider agreement and other provisions of this Service Policy.
We reserve the right to modify, limit, or exclude Optional Activity if, in the sole and absolute discretion of the Company or Companion Provider, Client-preferenced activity poses a potential problem or safety risk for the Client or Companion.
Escorted Outings
Escorted Outings are subject to a two (2) hour Service minimum (inclusive of door-to-door Client Transportation) and Client ability to move/ambulate with minimal assistance.
Clients and Companions are responsible for respective expenses. As a courtesy, Companion Providers may, if willing, temporarily fund nominal Client expenses. Expense receipts shall be obtained and submitted to Meriatrics LLC for Client billing and Companion reimbursement. Clients/Authorized Parties agree to pay all such expenses.
All provisions of the “Optional Activity” and “Transportation” sections of this Policy apply to Escorted Outings.
Pal Friday Services
"Pal Friday" (business service mark and registered NH trademark of Meriatrics LLC) refers to social companionship combined with personalized concierge-style assistance services, including but not limited to errands, appointments, shopping and in-home tasks, with Companion Provider and Transportation.
Clients may also opt to have Pal Friday Services performed on their behalf (unaccompanied), at a reduced service rate.
All provisions of the “Optional Activity” and "Transportation” sections of this Policy apply to "Pal Friday" Services.
Transportation
Transportation is provided only in conjunction with and as incidental to Services. No general taxi, livery, rideshare, or other transportation services are offered, provided or implied.
Clients must be able to move/ambulate sufficiently with minimal assistance to participate in Transportation-related Services.
Transportation is subject to Company approval, Companion Provider agreement and provisions of this Service Policy.
Service Rates
Services are billed by the hour. Minimum visit time is one hour. Time overages are pro-rated in 15-minute intervals, based on Service rate.
Standard rates apply to weekday Services, 9:00 AM- 5:00 PM. Premium rates apply to nights, weekends and holidays.

Direct Services $40/hour
Virtual Services $30/hour
Remote Services $20/hour
Escorted Outings $40/hour
"Pal Friday" Services $40/hour
"Pal Friday" (unaccompanied) $32/hour
Premium Rates
Standard Rates + 30%
10% discount for veterans/spouses. Other discounts may be available. Discounts may not be combined.
Tolls and/or mileage surcharges (.50/per mile) apply beyond a twenty (20) mile radius of Hampton NH.
Service rates subject to change.
Billing & Payment
All Services are currently private pay. We accept cash and checks (payable to Meriatrics LLC).
Standing Services (weekly services, booked/reserved in advance) are billed monthly. Payment is due within thirty (30) days of billing date. Interest on unpaid balances is charged at a rate of 1.5% per month.
For non-standing Services, payment is expected at time of Service.
Cancellation Policy
Scheduled Services booked in advance may be cancelled at any time via phone, text, or email. A surcharge may apply for cancellation of Service appointments, as follows:

A surcharge of $15 shall be due and payable if less than 48-hour notice is provided

Consideration extended for illness cancellations of less than 48-hour notice; not to exceed one surcharge-free cancellation per every five (5) completed Service visits.
For cancellation of booked Services paid in advance, a surcharge equal to 15% of total Service amounts cancelled shall be assessed and deducted from any refund due for Services yet to be rendered at time of cancellation.
Emergencies
During an emergency, as determined by and in the sole and absolute discretion of the Companion Provider, emergency 911 assistance will be sought first, followed by notification of Client’s emergency contact.
If Services are being rendered in a care facility, the Companion Provider shall immediately notify facility staff for emergency response (and 911, if applicable), followed by notification of Client's emergency contact.
Companion Providers are not permitted to drive a Client to the hospital or administer medication.
Prohibitions
The following are, in the sole and absolute discretion of the Company or Companion Provider, cause for postponement, cancellation or denial of Services:

Client smoking during Services (including tobacco products/vaping)

Aggressive/threatening behavior or abusive language by Client/Authorized Party

Appearance of Client illness or impairment by drugs or alcohol

Client refusal to wear a seatbelt during Transportation

Client/Authorized Party failure or refusal to comply with Company Policies
-
Concerns related to safety or security, as determined by Companion Provider
-
Excessive cancellation of Service appointments
We also reserve the right to postpone/cancel Services due to inclement weather, hazardous driving conditions, unsafe conditions at a service location (uncleared snow, ice, or debris), or unforeseen emergencies as determined by the Company or Companion Provider.
NO WARRANTIES OR LIABILITY
Clients/Authorized Parties assume all risks associated with Services, including sole risk, responsibility and liability for illnesses, injuries, and damages arising from/during the provision of Services, including any Transportation/travel, and further agree that under no circumstances shall Meriatrics LLC be liable for any direct, indirect, incidental, consequential, punitive, special exemplary, or any other damages arising out of Services.
Updates
We reserve the right to change or modify this Service Policy at any time without notice. Updates are indicated at the top of the Service Policy ("Updated") with correlating date.
